Coordination of Care to Residents

For those residents that do not have a primary care physician, coordination of care is offered through a collaboration with Vantage Healthcare, a local physician group. We also assist with access to outside providers such as:

• Psychiatry

• Hospice

• Podiatrist

• Eye Glass Clinics
